Requirements

At least 5 years of Advanced Practice Registered Nurse (APRN) experience in outpatient settings, particularly in hypertension and cardiovascular health.
Minimum of 2 years focusing on primary care or related subspecialties.
Proficiency with telehealth platforms and electronic health records (EHR) systems.
Experience delivering care virtually, keen to maintain a clinical nursing practice via remote consultations.
Familiarity with managed care frameworks, specifically STARS and HEDIS metrics.
Bilingual in English and Spanish is highly preferred.
Active multi-state nurse practitioner license, with readiness to secure additional licenses as necessary.
