Aledade is focused on driving better health outcomes through innovative support for primary care practices. As a Clinical Pharmacist for Patient Care & Innovative Solutions, you'll play a crucial role in enhancing the medication management process for both practices and patients.

Requirements

Doctor of Pharmacy (PharmD) from an accredited institution.
A valid and unrestricted pharmacy license in at least two states within our designated region, with a willingness to obtain additional licenses as necessary.
A minimum of three years of experience in community pharmacy practice, clinical pharmacy, or completion of a PGY1 Residency.
Demonstrated knowledge of medication adherence strategies and pharmacy quality measures.
Proficiency with Excel or Google Sheets; ability to learn new software platforms.
Strong analytical and critical thinking skills.
